The 2 pictures shown are the before and after of a fused switch in which the fuses were so loose, they basically overheated and melted down. When larger switches such as this type with 400-amp fuses are being used it pays to visually inspect the physical condition of the inner devices. The tell-tale sign of a problem will start with power that fluctuates on and off of actual equipment begins to fail. In this case a simple procedure of just tighten the fuses mounting screws could have prevented this loss of production time and replacement of any equipment.
